By submitting this form I am stating that
I have obtained all necessary permission for placing copyrighted materials on reserve. If the items placed on reserve are of my own creation, I have placed a copyright notice on the first page of the materials. I realize these materials may not be available to students until 5 days from the date of request.
Photocopied materials which are not of the instructor's own creation will be removed from Reserves after the current semester. Materials may remain on Reserve for more than one semester ONLY if a letter from the copyright owner granting a longer reserve period is given to the LRC.
